Yes, your authorised representative can collect the DIS booklet from the following address:

For a fresh DIS booklet, the authorised representative must submit the DIS request form (PDF) and the ID proof, both signed by all the account holder(s). For an additional DIS booklet, the representative must submit Annexure B, which can be found on the last page of the DIS booklet.
