## Short answer

Converting physical shares into demat mode requires physical signatures on forms and documents. When signatures are not possible due to old age or health-related reasons, contact the company's Registrar & Transfer Agent (RTA) for alternative documentation. The forms and documents required may differ for each RTA. However, medical certificates from a government hospital and a thumb impression of the shareholder are generally required to process the dematerialisation request.
